📝 Consent & Medical Information

 

By enrolling your child in the Seoul Sports program, you:
 

• Authorize Seoul Sports to provide any necessary medical assistance or hospital treatment in the event of accident or illness.

• Accept full financial responsibility for any associated medical costs.

• Permit Seoul Sports to share registration details with healthcare professionals for emergency treatment purposes.

• Confirm that your child is fit to participate in all activities and release Seoul Sports, its directors, and staff from any liability related to participation, except in cases of proven negligence.

• Are responsible for providing any required medication for your child, clearly labeled with their name, dosage, and instructions.

• Understand that children with infectious or communicable diseases (as identified by the Department of Health) may not attend sessions.

 

🛡️ Safety & Behavior

 

• Participants are expected to show respectful and safe behavior at all times.

• If a child consistently poses a safety risk or disrupts the session, Seoul Sports reserves the right to suspend their participation.

• Parents/guardians will be informed of any such incidents in writing.

 

⏰ Attendance & Supervision

 

• Parents/guardians are responsible for ensuring their child arrives on time and is picked up promptly after the session.

• Only individuals listed on the registration form may sign children in and out.

• Any updates to pickup authorizations must be submitted in writing to: info@seoul-sports.com

 

🚫 Allergen Awareness

 

• To ensure a safe environment, we ask that parents do not bring nuts or nut-based products to any session or event.

 

📸 Media & Promotion

 

• Seoul Sports may take photos or videos for use in promotional materials (e.g. website, brochures, social media).

• These materials will be used exclusively for Seoul Sports and will not be sold or shared with third parties.

• If you do not wish for your child to appear in photos or videos, please notify us in writing at: info@seoul-sports.com

💳 Billing & Payment

 

• Invoices will be sent to the email address provided at registration.

• Each invoice will include payment instructions and due dates.

• Late or unpaid invoices may result in your child’s participation being suspended until payment is received.

 

🎒 Lost Belongings

 

• Seoul Sports is not responsible for any lost or misplaced personal items, including clothing or electronic devices.

📬 Mailing List & Communication

 

• By registering, you agree to receive email communications from Seoul Sports about upcoming events and updates.

• You may opt out of future communications at any time.
