Consent and Medical Information

 

By enrolling your child in the Seoul Sports Academy Program, you authorize us to provide any necessary medical assistance or hospital treatment your child may require due to accident or illness. You understand and agree to bear full financial responsibility for any related expenses.

 

You grant Seoul Sports permission to share the information provided on the registration form with healthcare professionals for emergency treatment purposes.

 

Upon enrolling your child, you declare their fitness to participate in all activities and release Seoul Sports, its Director, and staff from any liability arising directly or indirectly from participation.

 

Seoul Sports is liable for personal injury, property damage, or loss only in cases of proven negligence by Seoul Sports, its Directors, or employees.

 

Parents/guardians are responsible for providing necessary medication for children with medical conditions or special needs, clearly labeled with the child's name, dosage, and instructions.

 

Children with infectious or communicable diseases, as identified by the Department of Health, are prohibited from attending classes.

 

Safety and Behavior

 

Participants must exhibit exemplary behavior at all times for the proper operation of the event. If a child consistently endangers others' safety or disrupts the event, Seoul Sports reserves the right to suspend their participation until further notice, with detailed incident notification to parents/guardians.

 

Attendance and Supervision

 

Parents are responsible for ensuring their child's punctual attendance and transportation to and from the event. Only authorized individuals listed on the registration form may sign children in and out of classes.

 

Notification of any changes in authorized individuals must be sent to info@seoul-sports.com.

 

Allergen Awareness

 

Parents are encouraged not to bring nuts or nut-based products to the event for the safety and wellbeing of all participants.

 

Media and Promotion

 

Seoul Sports may capture photos/videos for promotional purposes related exclusively to Seoul Sports. These media assets will not be sold to third parties and may be used in promotional materials such as flyers, brochures, the Seoul Sports website, and social media channels.

 

If you do not want your child's image to be used for promotional purposes, please provide written notice to info@seoul-sports.com.

Billing and Payment

 

Billing and payment communication will be conducted via email. Invoices for the Seoul Sports Football Program will be sent to the email address provided during registration. Payment instructions and due dates will be clearly stated in the invoice. Failure to settle payments on time may result in the suspension of your child's participation in the program.

 

Lost Belongings

 

Participants are responsible for their belongings, and Seoul Sports is not liable for lost items, including electronic devices. For inquiries about lost property, please contact info@seoul-sports.com.

 

Mailing List

 

By registering your child for Seoul Sports, you consent to join our mailing list for communication regarding future events and updates.
